Update to 0.405:

0.405   2015-02-14
    - fix RT#78527 - first_val/last_val in documentation
    - fix RT#102055 - ExtUtils::MakeMaker required version absurdly high
    - update README (deploy it as README.md now)
    - fix compiler issue for older/ansi-c89 compilers
    - remove local compat workarounds in favour for ppport.h
