Apple SDK 15.0 breakage (cannot find headers)

FYI, some people have run into a broken C++ compiler after upgrading the
SDK 15.0.  It will report things like 'iostream': File not found,
likewise for other standard headers.

The fix is to remove or rename /Library/Developer/CommandLineTools and
reinstall (e.g. xcode-select --install).
