Your hypervisor is xen-3, I guess ?

You want either:
disk = [ 'phy:/dev/vnd0d,0x301,w' ]
or
disk = [ 'file:/path/to/disk/image,0x301,w' ]
With you disk entry, the script will try to attach /dev/vnd0d to another
vnd device.
