Pull up following revision(s) (requested by bouyer in ticket #746):
        sys/dev/ic/ahcisata_core.c: revision 1.2
* Add bus_dmamap_sync calls for cmd structure.
* During initialization, use the right port index when setting up the
  physical pointers for a port. Fixes issue with non-contigous ports.
