Log Message:
Nothing uses shared -> exclusive upgrades any more, so remove the code.
This is good since they are effectively the same as ...

        lockmgr(&lock, LK_RELEASE);
        lockmgr(&lock, LK_EXCLUSIVE);

.. and therefore don't behave as expected.
