Log Message:
Clean up low locking for sa_upcall_getstate(). We now pass in
flags to indicate if we already have either the "event" and/or the
"interrupted" lwp locked. Otherwise, lock it.

Also, set l_mutex in sa_putcachelwp() so that the lwp waiting around
to handle an upcall has a valid mutex. Saves a locking-against-ourself
panic.
