Check in WIP of getting this code working with DIAGNOSTIC
assertions enabled. We are unrolling sleepq_remove() and spreading
it between sa_getcachelwp() and sa_switch(). The main issue is that
sleepq_remove() makes the thread runnable, but we need to do some work
between removing it from the queue (lwp cache) and making it runnable.

Fix typo in sa_newcachelwp() about sa being undefined.
