Log Message:
Add a new routine, sa_neverrun(). It is the routine a fresh
lwp is pointed at. It will only be run if the lwp exits before ever
getting used.

sa_newcachelwp(): Always include new lwps in p_nrlwps.

sa_putcachelwp(): mark sleeping lwps as LW_SINTR, since they can be
interrupted. processes now exit correctly.
