Log Message:
Accept filters:

- Remove remaining #ifdef INET.
- Avoid holding locks so we don't need to do KM_NOSLEEP allocations.
- Use a rwlock to protect the accept filter list.
- Make it safe to unload accept filter modules.
- Minor KNF.
