Log Message:
- Rename cpu_lookup_byindex() to cpu_lookup(). The hardware ID isn't of
  interest to MI code. No functional change.
- Change /dev/cpu to operate on cpu index, not hardware ID. Now cpuctl
  shouldn't print confused output.
