Log Message:
- enable schizo_dmamap_create() after fixing it
- clean up some debug code
- remove unused schizo_intr_map()
- setup our iommu state properly
- for now, implement a 'struct schizo_iommureg' and use it in schizo.
  XXX this should be merged with iommu*

interrupts are still not working but we get to scsi probe now.
