Move back to plain-text distribution documents

Autogenerated distribution documents from XML files provides very
little value and adds lots of complexity.  Just go back to plain-text
documents.

Still to do: reformat the documents by hand so that updates are easier.
Keeping the current generated format so that any changes before 0.10
is released can easily be propagated into the branch.
