Why has it got to? Use a script! I've got this in line in my "/etc/ttys":

tty00   "/etc/ppp/jehova"               unknown on secure

And the script looks like this:

#!/bin/sh
exec /usr/sbin/pppd $* 57600 194.123.65.89:194.121.202.132 defaultroute mtu 576 netmask 255.255.255.255 -bsdcomp connect $0.chat
