Same happens here (netbsd-4, T43p) with a UMTS card which is
cardbus0 -> ohci1 -> usb6 -> ubsa0 -> ucom0. There is also
a ohci0 -> usb5 on that card with nothing connected.
On removal of the cardbus card it often crashes the system
like above. I can reduce the likelyhood by enabling cardbus
debugging.
Sometimes it also wedges with something like:
