This one seems to be unrelated to the previous. I can't see a vnode

That would be:
        struct pmap *pmap = vm_map_pmap(&l->l_proc->p_vmspace->vm_map);

Can you also print:
*l
*(l->l_proc)
*(l->l_proc->p_vmspace)
l->l_proc->p_vmspace->vm_map

I wonder what could be NULL in there.
