ESTALE if NFS is involved.
However, ESTALE doesn't appear to be referenced on many such
man pages.

get ESTALE return from open, see no reference to it in the man page.
a spot check of a couple other man pages shows that it's
missing there, too.

go through and add ESTALE to appropriate section 2 and section
3 man pages.
