In fvwm95:
Fvwm95 uses icons on menus. These are defined without a path, but are fetched from the directory /usr/local/icons (its not documented where or how this is set, and I have not managed to find out).
However, the package stuff installs the icons into /usr/X11R6/icons. This is probably set int the makefiles for the various icon packages.

Install fvwm and icons from pkgsrc, then try to use a menu!

As a temporary fix:
ln -s /usr/X11R6/icons /usr/local/icons
Ideally, this should be part of the installation procedure (for at least one of X, fvwm, xicons).
In the long term, someone should decide which is the right path, and fix the pkgsrc code.
