typical implementations of VOP_LOOKUP() lookup a DOTDOT entry like:
1. search the entry in the directory.
2. unlock dvp.
3. find and lock vp using info from 1.
4. cache_enter().
between 2 and 3, the directory structure can be
changed by another thread.
in that case, step 4 will enter an already stale entry.
