This is not a bug; /var/spool/output/lpd is the default
directory used by lpd(8) if the printcap entry doesn't have
an explicit "sd" -- see the description for "sd" in printcap(5).

The examples in etc/printcap are all commented out; if a user
wants to use them it's their responsibility to ensure that
the spool directory specified by "sd" exists.
Notice that various entries (lp, hp, remote, varian, vp) have
a spooldir set, and non of them default to /var/spool/output/lpd.
