Hello Michael,
I applied the patch and tested the tape drive.
Both reading and writing all kinds of block sizes worked.

I've prepared a patch for the physio routine:
http://ftp.netbsd.org/pub/NetBSD/misc/mlelstv/kern_physio.diff
With this patch I can read and write tapes with e.g. 128 byte blocks.
