What system/platform are you running? I've seen similar problems from
fsck with ~1.4.1/i386 running on a 486dx4-100vlb, and ~1.4.1/m68k on a
Quadra 840AV (and there are open PR's, too), but 1.4.3 seems to be a
lot more stable on both.
