run "tar tfvz" on it and see what's inside.

Check that you have only one PKG_DBDIR (/usr/pkg/pkgdb, and not
/var/db/pkg).

Check for directories in PKG_DBDIR that are empty, or otherwise
irregular.

find / -name perl
and see if it is in /usr/local or some other place.

I am pretty convinced that something is wrong with your system.
