Perhaps you previously used a tool like ../pkgsrc/sysutils/dvd+rw-tools 
which supports a "growISOfs" feature that sort of mimics direct writing
though it's just a wrapper around mkisofs ?  You should be able to do
all the steps manually by

 1) copy current contents to temp dir
 2) add or subtract files from temp dir
 3) make a new ISO file system
 4) burn new ISO filesystem to to the optical disc

Years ago (2002 ?!) I made a hardcopy of the netbsd guide which covers
writing an image to a CD/DVD in section 19 ; probably the info is still
in the current version.
