 I had the same problem, and discovered that sometimes PAM_AUTHTOK_RECOVER_ERR 
is 
 used, sometimes PAM_AUTHTOK_RECOVERY_ERR. A temporary fix is to edit /usr/pkg/
 include/security/_pam_types.h:

 It's not a real solution but it might help.
