I've seen some of these myself.

They have something to do with UVM. I took the time to revert kernels and
eventually realized that it wasn't anything in the source that had changed,
but it happened when /sys/arch/alpha/conf was changed to build UVM kernels.

There isn't a way to write a Mach config file, but it's just a trivial edit
to /sys/arch/alpha/conf/std.alpha. (Although I do have a fix to config for
this.)
