One of the jumper pairs on rear of the Plextor PX-W4012S is labelled 
BLOCK.  The manual says:

BLOCK: Block size.  You may need to change this setting if you are 
working in a UNIX environment and want to boot from your PlexWriter.


I put a jumper across those two pins and tried booting the PWS again:

So, the "block size is not 512 bytes" warning went away but it still 
didn't work.  :(

I also tried booting an XP1000 from this CD using the CD drive which 
came with the machine, a Compaq CRD-8322B.  That's an IDE drive and 
doesn't have a jumper pair labelled BLOCK, but presumably it's 
configured to be able to boot from a 512-byte-block CD like this, but 
it didn't work either:
