This happens with a number of kernel errors. I don't know enough about
kloader to say why, but it is a byproduct of your problem not the cause.

It could be bad memory, but it could be other things also. Does it
always stop in ioflush? If so, it is unlikely to be memory, but could be
the disk or a kernel bug. Also, if the 16mb is really built in then the
32mb card would likely be an add on not a replacement and would not help.
