I just got the following panic on a moderately used NFS server, running
1.3.2:

vrele: bad ref count: type VDIR, usecount -1, writecount 0, refcount 1,
    tag VT_UFS, ino 62003, on dev 0, 12
panic: vrele: ref cnt

A trace from the debugger showed:
