it's an old WD8013, probably pulled from a telebit netblazer...  I use
one in my firewall/masq/router box.

There are two versions of the board, one with jumpers for everything,
and one with a jumper for the IRQ, and DIPs for everything else.  The
following are excerpts from a friend of mine (Joey Pruett) who has dealt
with more than his fare share of Telebit equipment.  ;)

* jumpered version:

"i'm not sure of the numbering of the jumpers, but if you go from left to
right while holding the board in the canonical direction, then it should
be like this:"

w1-8    240
w1-7    300
w1-6    280

w2-3    10
w2-2    11
w2-1    15

* DIP version:

switch 1:  (1 == on, 0 == off)
240/d0000 10111001
300/d0000 11101001
280/d0000 11011001

(The last four bits control the shared memory interface with the default
at d0000-d4000.  I haven't personally fiddled with this to figure out
how it changes depending on settings.)

w1 is irq:
w1-4 10
w1-3 11
w1-1 15

The standard settings for these boards in a NetBlazer (in case you're
curious) are:

en0 240 10
en1 300 11
en2 280 15

It looks like this board is supported under NetBSD as we0, at 280/d0000
IRQ 9...  good luck.
