Why is that. An out-of-the-box 4.0.1 should nicely support
"wsmouse". You cannot use your old XF86Config (from XFree86-3.3.x),
because several options have changed. Here is the relevant part of my
config:

No patch should be necessary. Maybe you were trying to use 'Driver
"wsmouse"' instead of 'Driver "mouse"' together with 'Option "Protocol"
"wsmouse"'?
