Also make sure your disk geometry really matches what's the BIOS use.
What scsi adapter do you use ?
You may need to reinstall a MBR boot code; try 'fdisk -i'.

/usr/mdec/installboot /usr/mdec/biosboot /dev/rsd0a
you need to use the character device.
