That sounds like the file /sbin/init is no longer there.  This could be
because of severe filesystem damage--confusion about which partition is
the root (it was able to mount, so if you've only got one filesystem,
this can't be the problem).  Try reinstalling base.

That's a bug in the installer.  The order that the installer shows
everything is the order that it's actually in the directory--it's not
sorted as 'ls' in NetBSD really does it.

That sounds like /var/run/utmp isn't being updated correctly for some
reason.
