If it's any help, I had *exactly* this same problem on my MacIIcx four days
ago, it ran 1.3.3 fine but booting 1.4.1 thought that the HD was ext2fd, it
turns out that as I had previously loaded linux and this must have left some
traces/residue behind in the partition or driver sections.
I solved the problem using a low-level format with Apple HD Setup, then
partition, then MKFS 45,
then installation.
