I just check the FTP pkgsrc repository, but can't find anything like  
that for PHP4 and NetBSD 4.0.

I am not really sure, but I think you could upgrade to PHP5 and use  
the packages that are here:
ftp://ftp.NetBSD.org/pub/pkgsrc/packages/NetBSD/powerpc/4.0_2009Q2/

There's a phpmyadmin package in the FTP site above. Since I've never  
used pkgsrc (incredible, huh?), I can't tell you if the dependencies  
are included...
