Hi,
Formatting a SCSI disk is actually quite easy. Here is something from
my notes (thanks Craig):

3 | Interleave (MSB) |
4 | Interleave (LSB) |

This is the format of the raw SCSI command which can be issued from
the EPROM monitor. Some of the information is not required -- I used
{0x04, 0, 0, 0, 1, 0} to format two drives successfully.
