Well, thanks for the kudos. Sorry about the doc flaws and  bugs.

I dont think we ever managed to test installation from a CD or CD
image on a pmax.  It did get tested for an i386, but not the old pmax
device drivers. (Heavy sigh) so we didnt catch this particular
problem.

Terry: could you please send PRs for *each separate point*, (i.e., one
for each) so's they don't get lost and they all get fixed?
