Here's my short form for attempting to solve this:
1. Unplug *everything* except the modem.
2. Replace internal house wiring.
This will solve 90% of the poor line quality problems out there.
Some people have phone devices that put multi-farad capacitors
across the line even when on hook, and many of the rest have barbed
wire for interior cabling.
