I deal with these frequently commercially on PDP11's. I have found that
about 20% have had their firmware changed to something other than default.
Either use the console and access the firmware and reset it to default or
hook up a terminal. IIRC the command is Z to reset to default. This is
several menus deep however. See capture below from a controller that came
back from a dealer but was good. It just needed to be reset to default.
Frequently I find LUNS set to various # and some drives partitioned.
If you wish I will scan a few pages from the manual that describe getting
into the firmware without using the 10 pin connector from a VAX.
