Don't do _BSD_SIZE_T_ dance here. Use <stddef.h> in userland.
This file is used in tools, where _BSD_SIZE_T_ is unreliable. In the
kernel, <sys/types.h> will define size_t anyway. Outside the kernel,
<stddef.h> is the standard way to get size_t.
