Various formatting changes:
* Remove superfluous Pp macro (fixes mandoc(1) warning)
* use Xr macro instead of Nm where appropriate
* escape punctuation (so Ic actually 'sees' it) (fixed mandoc(1) warning)
* remove redundant argument to successive Nm macros
* remove double spaces
