Why did you add these two lines?!

asc0    at      tc? slot ? offset ?     # NCR53C94 SCSI
le0     at      tc? slot ? offset ?     # LANCE

There are already:

asc*    at      tc? slot ? offset ?     # PMAZ-A single channel SCSI
le*     at      tc? slot ? offset ?     # PMAD-A LANCE

in the config file!
