For NetBSD, it's a nonstandard place, as you say, so they should be
able to change some config file(s). What I disliked with the previous
state was that makewhatis failed in the _default_ installation,
because we do not have /usr/X11, while we do provide the other
necessary directories (if you install X).

But perhaps a better solution would be to teach makewhatis not to
despair because a directory doesn't exist...
