Log Message:
- Use PMAD-BA name for device match logic.   It's not a real TURBOchannel
  device and the id string is not found in option ROM string, but
  ULTRIX/OSF1 call it so anyway.
- Squash IOASIC DMA buffer allocation into device attach() sequence.
