Log Message:
Make pthread__upcall() non-static; it didn't really serve any purpose
for it to be static, and made debugging more annoying.

Wrap counters in #ifdef PTHREAD__DEBUG, so that libpthread builds
when that's not defined.

Add a little bit of debugging information.
