significant clean up effort, largely from petrov@ via martin@.

- fix a bunch of comments for proc->lwp
- remove old TRAPTRACE, NOT_DEBUG, etc. code that has bit-rotted
- put some older DEBUG code under NOT_DEBUG
- add a bunch of new debugging/tracing code ("KTR") that depends on
other changes not yet present.
