I believe the genesis of all these is Thomas Anderson's PhD work
(Univ. of Washington, around 1990/1991/1992) on Scheduler
Activations.
See SOSP (Symposium on Operating System Principles) or TOCS
(Transaction
of Computer Systems, I think) from those years. The kernel
cooperates
with the user scheduler to let it know which execution contexts are
really running, and the user level can map that to whatever logical
threads it wants.
