I've figured out the problem:

cisco-sw1#show interface fastEthernet 0/4 switchport

Operational Trunking Encapsulation: dot1q
Negotiation of Trunking: On

Trunking Native Mode VLAN: 82 (CogentMgmt)

Note vlan 2 vs 82.

so, this explains why the traffic was arriving both tagged and untagged,
and clearly this confused everything.  Some packets would sometimes go
through and then stop.
