This looks very fine to me.
> # pkg_delete -v mutt

Checking the ktrace output, this should just not happen.
Do you have any funky mount options in effect, that prevent executing
things on /var?
What does
* mount
* cd /var/db/pkg/mutt-* ; ./DEINSTALL
say?
