You can try setting this in ~/.config/gtk-3.0/settings.ini:

[Settings]
gtk-button-images = true
gtk-menu-images = true

From what I can tell, the default in Gtk+ 3 is now without any icons in 
menus or buttons. They got some kind of purity plague apparently.
