I'm experiencing similar with postfix 2.6.5 from pkgsrc HEAD on a
5.0_STABLE sparc64 system:

It only seems to happen with TLS enabled, but on both sides of the
connection (both systems in the log snippet above are 5.0_STABLE sparc64
running the same version of postfix). I don't have libmilter enabled, but
do use the following:
PKG_OPTIONS.postfix = bdb pcre sasl tls
As in your case, my problems only started after a recent postfix update -
before that, TLS had been working like a charm for years.
